1.FOR FIRST REGISTRATION

Two sets of self attested photocopy of  following documents are required .

1. 1.Marksheet of High school.
   

1. 2.Marksheet of Intermediate .
   
2. 3.Mark sheet of Diploma/Degree in Pharmacy of all the years
   
3. 4.Certificate of High school.

5. 5.Certificate of Intermediate.

7. 6.Diploma / Degree of Pharmacy or Provisional certificate issued from the college showing passed Diploma/Degree in pharmacy examination
   
8. 7.Practical Training certificate of 500 hrs from PCI recognized Government Semi Government and Government aided Hospitals for Diploma Pharmacy candidate only.
   

1. 8.Current Domicile Certificate ( issued within six month ) For those condidate who are passed from outside of UP.
   
2. 9.Six Coloured Passport Size Photograps.

4. 10.Rs. 500 Registration Fee, Rs.200 Development Fee.

Note:  Kindly  bring  original copy of all documents for verification.

2. RENEWAL

The following set of self attested Photocopy are Required.

1. 1.Green Card holder are required to bring recent coloured passport size photographs.
   
2. 2.For those ,Applying for Green Card, two recent coloured passport size photographs to submitted.
   
3. 3.Green Card  in orginal issued by UP Pharmacy Council.
   
4. 4.Photocopy of Diploma/Degree(if not deposited)

Renewal Fee

1.Rs. 500/- for Five Years

2. Rs. 0/-  for those Pharmacist who are Registred under  Life Renewal Scheme.

3. FOR TRANSFER CASES

1. 1. Self attested two photocopy  Marksheet and Certificate/Diploma/Degree of High School , Intermediate, B.pharma.
   
2. 2. U.P. State Domiciles Certificate (issued within six month)
   
3. 3.Registration Certificate and Renewal Certificate (in original) issued by other state's pharmacy council where pharmacists is currently registred fee Rs. 2000.

4. ENTRY OF ADDITIONAL QUALIFICATION.

1. 1. Registration Certificate and Renewal Certificate (Green Card) in original.
   
2. 2.Two self attested photocopy of Marksheet and Degree of additional qualification along with original document for verification fee Rs. 100.
