Rs. 0/-  for those Pharmacist who are Registred under  Life Renewal Scheme. 3.LATE FEE RENEWAL/RESTORATION a.Within Ten Years Rs.200/-. b.More Than Ten Years Rs.500/- . c.Annual Renewal Charge for Per Back Year ( 1990  Rs. 5/- per annum,1991 to 1999 Rs. 10/- Per annum , 2000 to 2005 Rs. 40/- per annum , 2006 to Till Date Rs. 100/- Per annum).   4.TRANSFER/RECIPROCAL Rs.2000/-. 5. ENTRY OF ADDITIONAL QUALIFICATION. Rs.100/-. 6.*Duplicate copy of Registration certificate Rs.1000/- . 7*.Duplicate copy of Green Card Rs.300/-. * For issue Duplicate Copy of Registration Certificate /Green card The condidate must have provide the following documents. 1.Self attested photocopy of Diploma Certificate/Degree/Registration Certificate issued by UPPC if (Available).. 2.Three Coloured Passport Size Photograps. 3.An Affidavit on non judicial stamp paper. 4.News Paper Cutting ( Publish in news paper ). 5.FIR Copy of Police Station.
